In this video we present a two-year-old female with a febrile urinary tract infection and E. coli bacteremia who presents with suspected parenteral diarrhea, a non-infectious diarrhea associated with an extra-intestinal infection. The concept of parenteral diarrhea is over a century old and generally accepted in the literature but there have been no high-quality studies to prove whether or not it is a real clinical entity. So what? Why does it matter. It matters because vomiting and diarrhea occurring as a non-infectious side effect of a urinary tract infection can cause diagnostic confusion and failure to diagnose a potentially serious illness.
