She was treated with IV antibiotics which normalised her blood picture but she continued suffering with pelvic pain. On examination, she had a soft abdomen but was tender on vaginal palpation. An ultrasound revealed a left hydrosalpinx. She was assessed by HSG which did confirm the presence of left hydrosalpinx as well as a distended right tube. However, spillage of the radio opaque dye was noticed from that tube. She consented to an explorative laparoscopy

She was admitted with severe abdominal pain at 36 weeks of gestation after a fainting episode at home. Emergency ultrasound showed free fluid in the abdominal cavity with tonic uterine contraction. She underwent emergency cesarian section during which a large amount of free blood spilled trough the peritoneal incision. A live fetus was delivered in good condition. The low incision was closed, and the uterus extracted and inspected. There was a large rupture in the fundus where the fundal myomectomy had been carried out. This was repaired, and the patient recovered uneventfully. In last week’s video, we showed a straightforward technique for a total laparoscopic hysterectomy (TLH) with sacroiliac ligament (SIL) vaginal suspension in a woman with a second degree uterine prolapse and a cystocele. Today, we will demonstrate a similar method in a patient with severe large bowel adhesions and pelvic inflammation due to large bowel diverticulitis. Case The patient is a 62-year-old woman with two previous cesarian sections, an appendectomy and a long standing history of pelvic pain due to large bowel diverticulitis treated with IV antibiotics. She had a failed colonoscopy due to narrowing of the sigmoid lumen as a result of adhesions and inflammation.

However, she developed low abdominal pain the night before her procedure. During laparoscopy, she had suffered right adnexal torsion. The ovary was still salvageable but due to her decision to undergo oophorectomy, a typical right adnexectomy was performed.
